The breeder gives you the health form from the vet.You need to also go to the vet if you are selling pups and guaranteeing the health.Unless you are a skank bone head breeder then you can just make one up on your computer like the rest of them do. But they are worth only as much as the paper it's printed on.
Each breeder has their own health form and they also have their vets sign it and they sign it too. You can look on the AKC sight for a form to copy but If your getting a pup from a breeder they will have the form already. If they dont then they are not a resposible breeder in the first place.
